Ways to Wellness - Adult 12-Week Program

The Ways to Wellness weight management program for adults includes:

  • 2 - wellness assessments
    • metabolic testing
    • body composition (body fat, BMI, waist/hip ratio)
    • fitness evaluations (strength, flexibility, cardio-respiratory, endurance)
  • Individualized visits with your Wellness Coach
  • Comprehensive wellness plan with personalized goals, including a diet and exercise plan based on your wellness assessment
  • Workbook and food and activity log
  • Complimentary two-week pass to an area fitness club Apple
  • Nutrition counseling
  • Personal training
  • Classes that focus on nutrition, fitness and body image

Cost of the Adult 12-week program: $449

2.  Fill out the registration packet

Before your appointment, please complete the forms in your packet. Please read through the materials carefully and completely. They include:dragonfly

  • Registration, payment options and cancellation policy
  • Liability
  • Medical history. Please fill out the medical history form completely. If you have indicated any of the health concerns for numbers (1) through eight (8) you will need to get your doctor's approval.
  • Heart disease risk factors. If you check 2 or more, you will need your doctor's approval.
  • Medical release. If you need your doctor's approval, bring this form to your clinic.
